For Families Living With a Host Family:

Students residing in a home that is not owned by their parent or guardian are considered to be “hosted.” There is one form for the parent/guardian to complete letting the Registration Office know that a student is living with another family. The second form is for the homeowner/leaseholder (also called “host family”) to complete confirming that you are residing with them at their residence. Together these forms are verification that you, your student, and the homeowners/leaseholders are all living together at the same address. Both forms must be signed and notarized before submission.

There are two forms required to verify this arrangement:

  1. Host Affidavit - Parent/Guardian:* Completed by the parent or guardian to inform the Registration Office that the student is living with another family.
  2. Host Affidavit - Host:* Completed by the homeowner or leaseholder (the “host family”) to confirm that the student is residing at their address.

If you are being hosted in a rental: Complete the online landlord affidavit. The landlord affidavit should be completed by your landlord and/or property manager verifying your rental details. This form will confirm both the current lease term and occupants of the rental property. This should be submitted with your host documents.
