For Families Living With a Host Family:
Students residing in a home that is not owned by their parent or guardian are considered to be “hosted.” There is one form for the parent/guardian to complete letting the Registration Office know that a student is living with another family. The second form is for the homeowner/leaseholder (also called “host family”) to complete confirming that you are residing with them at their residence. Together these forms are verification that you, your student, and the homeowners/leaseholders are all living together at the same address. Both forms must be signed and notarized before submission.
There are two forms required to verify this arrangement:
- Host Affidavit - Parent/Guardian:* Completed by the parent or guardian to inform the Registration Office that the student is living with another family.
- Host Affidavit - Host:* Completed by the homeowner or leaseholder (the “host family”) to confirm that the student is residing at their address.
If you are being hosted in a rental: Complete the landlord affidavit. The landlord affidavit should be completed by your landlord and/or property manager verifying your rental details. This form will confirm both the current lease term and occupants of the rental property. This should be submitted with your host documents.
This form is fillable when downloaded. Once completed, you must save a copy of the form to upload to Genesis in step 2.
